When it is not a dentist you need, but emergency care
If any of the following applies, seek urgent medical assessment through the emergency services rather than waiting:
- difficulty breathing or swallowing
- swelling of the face or neck that is getting worse quickly
- heavy bleeding that cannot be controlled
- severe facial injury
- feeling generally unwell (high fever, confusion)
In Italy the single emergency number is 112, free from any phone.
A tooth that breaks, a crown that comes off, pain that starts during the night, an orthodontic wire that digs in, a denture that fractures: dental emergencies vary widely. The aim is to identify the problem quickly, bring pain and discomfort under control, and decide on the most appropriate solution.
The most common dental emergencies
Not every emergency needs the same treatment. A clinical assessment shows whether a temporary solution is enough, or whether something needs doing straight away.
Pain may be caused by deep decay, inflammation of the pulp, infection, an abscess, gum problems or other conditions. The purpose of the visit is to identify the cause: simply switching off the pain without a diagnosis can postpone a problem that keeps developing.
A fracture may involve the enamel only, or extend to dentine, pulp or root. Even when there is no pain it is worth having the tooth assessed quickly, to establish whether it can be rebuilt and protected.
If a crown, veneer or bridge comes away, keep it and bring it with you. Where the clinical conditions allow, it may be possible to re-cement it; in other cases the tooth or the restoration will need work.
A partially erupted wisdom tooth can cause pain, inflammation of the surrounding tissue, difficulty opening the mouth and swelling. A clinical assessment, with X-rays where indicated, allows the most appropriate management to be defined.
Swelling of the face or gum, throbbing pain, discharge or fever call for particular attention. It is important to assess the origin of the infection promptly and establish the dental treatment required.
A removable or temporary denture that breaks can affect chewing, speech and comfort. We assess whether it can be repaired, temporarily adjusted, or replaced with an alternative solution.
Movement of a crown, a screw or a prosthetic component on an implant should not be ignored. An early assessment can prevent a mechanical problem from getting worse.
After orthodontic treatment the retainer helps hold the teeth in the position they have reached. If it is lost, breaks or no longer fits properly, it is best not to wait too long, to limit any movement.
An archwire can shift, come away from a bracket, or irritate the cheek and soft tissue. We can reposition it, shorten it or stabilise it depending on the situation.
A detached bracket can interfere with treatment and cause discomfort. Whether it needs repositioning immediately depends on the stage of treatment and on the symptoms.
If an aligner is lost or breaks, it is better to get advice before moving to the next tray on your own: the right choice depends on the stage of the treatment.
After a fall or a blow, a tooth can fracture, be displaced or be knocked out entirely. In dental trauma time can be decisive: contact the practice as soon as possible and describe what happened.
Losing a filling can leave the tooth sensitive and more vulnerable. An assessment shows how much tooth structure remains and which restoration is indicated.
During prosthetic, implant or restorative treatment a temporary can come loose or fracture. Restoring it protects the tissue and helps maintain function and appearance during the course of treatment.
What to do while you wait
Do
- Call the practice and describe the symptoms, when they started and what happened.
- If a crown, veneer, denture or fragment of tooth has come away or broken, keep it and bring it with you.
- If there is bleeding after an injury, apply clean gauze with gentle pressure.
- If a permanent tooth is knocked out completely, contact a dentist immediately: management is time-dependent.
Better not to
- Do not glue crowns, veneers or fragments back yourself with household adhesives.
- Do not take antibiotics left over from a previous course without a prescription for this specific episode.
- Do not put off the assessment because the pain has gone: the cause can keep developing silently.

For an over-the-counter painkiller, follow the instructions on the packet and, if you have any doubts or are taking other medication, ask a doctor or pharmacist. What matters is not to take antibiotics left over from a previous course: they need to be prescribed for the episode you have now.
It is not an emergency, but it should not be left for long either. The tooth underneath is exposed and can be more vulnerable, and neighbouring teeth can drift, which makes re-cementing the same crown harder. Keep the crown and bring it with you.
